SCOPE OF WORK <br /> Well Destructions for California Tank Lines,3105 South El Dorado Street, Stockton CA <br /> Central Valley Regional Water Quality Control Board Case#390541 <br /> California Global ID#T0607700425 <br /> On behalf of El Dorado Land Holdings, LLC, the owner and Responsible Party (RP) for the above referenced <br /> project, Innovative Environmental Solutions (IES) is requesting a Site Mitigation Well & Boring Permit for <br /> the destruction of off and on-Site wells. This request is based on the directive outlined by the Central Valley <br /> Regional Water Quality Control Board (CVRWQCB) in its Well Destruction Report Request letter dated 21, <br /> April 2025. The following is the scope of work for the well destructions: <br /> 1. Create and implement a Site-specific Health and Safety Plan (HASP) outlining hazards associated <br /> with drilling and well destructions; <br /> 2. Prepare and submit an encroachment permit application to City of Stockton(City)to allow access for <br /> the destruction of thirteen(13) groundwater monitoring wells located on City property (MW-4, MW- <br /> 6, MW-7, MW-8, MW-12, MW-13A, MW-1413, MW-1513, MW-16C, MW-17B, MW-18C, MW- <br /> 19B and MW-20C); <br /> 3. Prepare and submit a permit application to San Joaquin County Department of Environmental Health <br /> (DEH) for the destruction of twenty-one (21) on and off-Site groundwater monitoring wells (MW-1 <br /> through MW-9, MW-10A, MW-1113, MW-12B, MW-13A, MW-14B, MW-1513, MW-16C, MW- <br /> 17B, MW-18C,MW-19B,MW-20C and EX-1); <br /> 4. Notify Underground Services Alert (USA)to ensure that all proposed drilling locations are free from <br /> subsurface utilities; <br /> 5. Direct and oversee the destruction of twenty-one (21) on and off-Site groundwater monitoring wells <br /> at the locations depicted on Figure 1; <br /> 6. Manage the removal of all investigation derived waste, including remediation system component <br /> materials, in accordance with all state and local requirements; <br /> 7. Prepare and submit a well destruction report detailing all well destruction and remediation equipment <br /> sealing and removal procedures. <br /> Each part is described in the following sections. <br /> Safety and Health Plan <br /> IES has established a Safety and Health Program(SHP)to ensure the personal health and safety of all IES <br /> employees. The SHP will define safety practices and procedures to be instituted in all IES work places, as <br /> applicable. The program meets, and often exceeds, the requirements promulgated by the Occupational <br /> Safety and Health Act (OSHA). As part of the SHP, all IES personnel are appropriately trained and under <br /> a Medical Surveillance Program in accordance with OSHA 40 CFR 1910.120. <br /> IES's primary mechanism to ensure employee, environmental, and public safety at the project site will be <br /> the site Health and Safety Plan (HASP). IES will prepare and implement a HASP for this project. All <br /> individuals working under the purview of IES will be required to follow the health and safety information <br /> contained within the HASP. The HASP will be site-specific and task-specific, describing hazardous <br /> conditions that may be encountered and prescribing the necessary safety protocols to protect employees <br /> from these hazards. The HASP will be reviewed by the project management team and then reviewed and <br /> approved for field use by the Site Safety and Health Officer(SSHO). The HASP will be implemented and <br /> enforced on site by the assigned SSHO. IES will prepare a comprehensive HASP based on the scope of <br /> work and the potential hazards described in this work plan. <br /> At a minimum, the HASP will identify the following: roles and responsibilities of key site personnel; <br /> hazard analysis for all chemical, physical, and physiochemical hazards anticipated; a personnel protection <br />
