The precise location shall be determined in the field by the City of Stockton. In no case shall any monitoring well be installed within a travel lane. 3. The issuance of this permit by the City does not create any agency on the part of the Permittee, and the permittee will hold the City of Stockton harmless for any action for damages arising out of the Permittee's activities performed under this permit. 4. In accordance with Section 1(h) of the "Standard Conditions Applicable to Revocable Permits,"the permittee shall provide and keep in force during the term of the permit a policy of public liability insurance against liability for injuries to persons or property arising out of accidents attributable to the structure on City property, in amounts to be determined by the Risk Manager. The policy of insurance so provided shall contain a contractual liability endorsement covering the liability assumed by the Permittee by the terms of his permit and shall contain a provision that such policy may not be canceled except after thirty(30) days notice in writing to be given to the Risk Manager. The copies of these policies or certificates evidencing the same shall be filed with the City Risk Department after approval of the City Attorney. 5. Permittee, or his contractor, shall obtain an Encroachment Permit for the installation of the monitoring well. 5. Future monitoring well sampling, maintenance, and/or removal shall require an additional Encroachment permit. 6. Work site must be marked with location of all underground utilities via the Underground Service Alert prior to the required field meet with Public Works. 7. The well head shall be covered with a meter box or valve box as approved by the City of Stockton. The top of the box shall be flush with the surrounding ground or concrete. 8. The provisions of the State of California, Department of Water Resources Bulletin 74-81 and 74-90 entitled "Water Well Standards: State of California" shall be applicable to this permit. 9. Construction equipment that emits loud noise shall use muffling device. Construction shall be confined between the hours of 8:30 a.m. and 5 p.m.
