Revocable Permit No. RP23-0892 <br /> INNOVATIVE ENVIRONMENTAL SOULTIONS <br /> Page 2 <br /> 2. The general location of the monitoring well shall be as shown on the map attached. <br /> The precise location shall be determined in the field by the City of Stockton. In no <br /> case shall any monitoring well be installed within a travel lane. <br /> 3. The issuance of this permit by the City does not create any agency on the part of <br /> the Permittee, and the permittee will hold the City of Stockton harmless for any <br /> action for damages arising out of the Permittee's activities performed under this <br /> permit. <br /> 4. In accordance with Section 1(h) of the "Standard Conditions Applicable to <br /> Revocable Permits,"the permittee shall provide and keep in force during the term of <br /> the permit a policy of public liability insurance against liability for injuries to persons <br /> or property arising out of accidents attributable to the structure on City property, in <br /> amounts to be determined by the Risk Manager. The policy of insurance so <br /> provided shall contain a contractual liability endorsement covering the liability <br /> assumed by the Permittee by the terms of his permit and shall contain a provision <br /> that such policy may not be canceled except after thirty(30) days notice in writing to <br /> be given to the Risk Manager. The copies of these policies or certificates <br /> evidencing the same shall be filed with the City Risk Department after approval of <br /> the City Attorney. <br /> 5. Permittee, or his contractor, shall obtain an Encroachment Permit for the installation <br /> of the monitoring well. <br /> 5. Future monitoring well sampling, maintenance, and/or removal shall require an <br /> additional Encroachment permit. <br /> 6. Work site must be marked with location of all underground utilities via the <br /> Underground Service Alert prior to the required field meet with Public Works. <br /> 7. The well head shall be covered with a meter box or valve box as approved by the <br /> City of Stockton. The top of the box shall be flush with the surrounding ground or <br /> concrete. <br /> 8. The provisions of the State of California, Department of Water Resources Bulletin <br /> 74-81 and 74-90 entitled "Water Well Standards: State of California" shall be <br /> applicable to this permit. <br /> 9. Construction equipment that emits loud noise shall use muffling device. <br /> Construction shall be confined between the hours of 8:30 a.m. and 5 p.m. <br />
