4. Clear utilities, drill and install six groundwater monitoring wells, three in the B horizon and <br /> three in the C horizon, to further define the lateral and vertical extent of impact from petroleum <br /> hydrocarbon range constituents in subsurface groundwater; <br /> 5. Collect soil samples in five-foot intervals for classification by the United Soils Classification <br /> System (USCS) and field screening using a hand-held photo-ionization detector. Soil samples <br /> indicating potential source contamination during field screening will be submitted for laboratory <br /> analyses including TPHg, TPHd, TPH-MO, BTEX, fuel oxygenates MTBE, DIPE, ETBE, <br /> TAME, TBA, naphthalene, 1,2 dichloroethane (1,2-DCA), ethylene dibromide (EDB), and total <br /> lead. Any soil samples containing detectable concentrations of TPH-MO will be additionally <br /> analyzed for VOCs, semi-VOCs, and title 22 metals. <br /> 6. Survey the newly installed groundwater monitoring wells to a municipal benchmark. The <br /> location and elevation of measuring marks at the wellhead will be surveyed to the nearest 0.01 <br /> foot by a licensed land surveyor; <br /> 7. Gauge, purge and sample all groundwater monitoring wells associated with the site (MW-1; <br /> MW-3 and MW-4; MW-6 through MW-20C). Continue to sample and analyze all groundwater <br /> monitoring wells for TPHg, TPHd,BTEX,MTBE,DIPE, ETBE, TAME, TBA, naphthalene and <br /> 1,2-DCA, on a semi-annual basis. Recommended groundwater analyses may alter based on the <br /> results of this proposed assessment; <br /> 8. Prepare and submit to the RWQCB an additional site assessment report that includes an updated <br /> site conceptual model and provides recommendations with respect to the Low-Threat Closure <br /> Policy(LTCP). <br /> Each part is described in further detail in the following sections. <br /> 5.1 Health and Safety Plan <br /> IES has established a Safety and Health Program(SHP)to ensure the personal health and safety of all IES <br /> employees. The SHP defines safety practices and procedures to be instituted in all IES work places, as <br /> applicable. The program meets the requirements promulgated by the Occupational Safety and Health Act <br /> (OSHA). As part of the SHP, all IES personnel are appropriately trained and under a Medical <br /> Surveillance Program in accordance with OSHA 40 CFR 1910.120. <br /> IES's primary mechanism to ensure employee, environmental, and public safety at the project site is the <br /> Health and Safety Plan (HASP). IES will update the existing HASP for this project to address the <br /> additional hazards associated with work in traffic zones. All individuals working under the purview of <br /> IES will be required to read and sign the HASP to acknowledge their understanding of the information <br /> contained within. The HASP is site-specific and task-specific, describing hazardous conditions that may <br /> be encountered and prescribes the necessary safety protocols to protect employees from these hazards. <br /> The HASP will be reviewed by the project management team and then reviewed and approved for field <br /> use by the Site Safety and Health Officer.
