You should receive the check in the mail in a few days. I understand your need to complete the fieldwork prior to the end of this fiscal year but unless our agency determines that that two additional wells are needed, there is no need to submit the permit application and fee. Nuel Henderson and I have checked our work schedules for the next week and the earliest that I can give you a definite answer concerning approval of the proposed fieldwork is 10 June 2015. If the proposed work is approved by our agency, I can do the inspections on June 171h through June 19t; however, I am not available until 2:00 p.m. on June 171h If nothing has changed on the Master File Record (MFR)form that was submitted in November 2014, then there is no need to submit an additional MFR. I am appending the Well and Boring Permit Application and the Licensed Contractors' Declaration form. You must complete these forms and submit a check for$260 if two monitoring wells are approved by our agency. Thank you, Greg. Actually our attempt to submit a permit application is in response to conversations we have been having with Nuel. My geologist here prepared the permit based on the most recent I
