The permit has been approved with the following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control District <br />for applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. All new addendums supercede the previous ones. <br />3. At least one person who holds the International Code Council (ICC) "UST <br />Install/Retrofitting" certification and manufacturers' certificate of training for all <br />components installed or tested must be onsite at all times when work is performed. <br />4. Each person installing or testing any component must hold a certificate of training from the <br />manufacturer of that component. <br />5. The "Certificate of Compliance for Underground Storage Tank Installation Form C" shall be <br />submitted by the owner or the owners' agent [pursuant to California Code of Regulations <br />(CCR) 2635 (d)] before this upgrade will be considered complete by SJCEHD. <br />6. Concrete will be broken above the tank pit, a tank integrity test must be performed by a <br />certified tank tester and results received by this office within 7 days of completion of work. <br />7. Changes to scope of work may require additional plan review and inspection(s), which can <br />result in additional fees. <br />8. Contact Toua Yang at (209) 468-3459 to arrange the following inspection appointment(s); <br />a 48-hour notice for inspection appointment(s) is required. <br />a. EHD must verify the overfill prevention system is in compliance with CCR 2635 <br />(b) (2). Note: Your company may choose to have EHD onsite during the overfill <br />prevention system installation so that portions of the system do not have to be <br />disassembled/reassembled. <br />9. Permit expires 90 days from the approval date.
