Financial responsibility documents have not been submitted to the Environmental Health Department (EHD). Current financial responsibility documents are required to be submitted annually. The EHD received the documents during the inspection. 5. The monitoring plan on site is not current. The monitoring plan must be current and approved by the EHD. Complete and submit a copy of the monitoring plan for aproval by the EHD by April 23, 2011. An approved copy will be returned for your records. y D-X9i 7. The alarm log entry and maintenance invoice was not found on site for the C1 - 91 octane tank annular sensor "sensor out alarm" on 1-17-11. These records shall be kept on site for at least 3 years. Immediately locate and maintain the above documents on site and submit copies to the EHD by April 23, 2011. 12 & 16. The buried 91 octane and diesel product secondary containment lines between the transition sump and the dispensers were not open to flow into the transition sump. Any potential release from the primary containment into the secondary containment in the 91 and diesel piping does not flow into a collection sump. All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the low point of the underground piping. The monitoring certification technician removed the valve caps and Schrader valves from both lines and corrected the violation during the inspection. 23c. The designated operator failed to document all the alarms from the attached alarm history on the 1/11 and 2/11 designated operator monthly inspection reports and failed to check that they were responded to appropriately. The missing alarms include: C1-91 annular sensor sensor out 1-17-11, sensor out 12-20-10 and fuel alarm 12-17-10. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
