The permit has been <br />approved with the following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br />District for applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. <br />3. At least one person who holds the International Code Council "UST Service <br />Tecnician" certification must be onsite AT ALL TIMES when work is performed. <br />4. Manufacturer's certification of training and ICC Service Technician certification <br />must be presented at time of appointment before any testing can take place. <br />5. Contact Lori Luces at (209) 468-0341 to arrange the following inspection <br />appointments; a 48-hour notice for inspection appointments is required. <br />a. Leak Detector Test: leak detector must be tested to confirm that the device <br />detects a leak of 3 gallons per hour at 10 psi. <br />6. Permit expires 90 days from the approval date. Note: if an extension to the <br />expiration date is required, this office prior to the expiration date must receive a <br />written request. <br />Thank you, <br />4�- <br />Lori 1.
