Elite IV Contractors <br />6649 Embarcadero Rd., <br />Stockton CA 95219 <br />Re: Village West Marina @ 6649 Embarcadero Rd., Stockton <br />The permit application to explore the monitoring system failures at the above referenced site <br />has been approved with the following conditions: <br />1. A complete description of work performed must be submitted prior to repairing or <br />replacing any portion of the monitoring system. <br />2. All monitoring equipment used must be listed in LG -113. <br />3. If any piping is to be repaired or replaced or if concrete is broken over underground <br />piping, Environmental Health Department (EHD) must be notified so that an <br />inspection can be arranged. Soil sampling, may be necessary per EHD assessment. <br />Additionally, all required inspections related to a piping retrofit must be scheduled <br />with this office. Per EHD assessment of findings, new plans and a permit application <br />may be required to be submitted. <br />4. Manufacturers specification sheets including instructions for the repair must be <br />submitted prior to beginning the repair. Per EHD assessment of findings, new plans <br />and a permit application may be required to be submitted. <br />5. If sumps or dispenser pans are repaired/replaced a 24-hour lake test will be required. <br />6. If concrete is broken near the underground storage tank, then a 3rd party tank integrity <br />test must be performed once the site is returned to operating conditions. Results must <br />be received by EHD within 7 days of returning the site to operating condition. <br />7. If concrete is broken near the underground piping, then a 3rd party line integrity test <br />must be performed once the site is returned to operating conditions. Results must be <br />received by EHD within 7 days of returning the site to operating condition. <br />8. Notify EHD of all third party test dates. <br />9. Notify EHD once work is complete.
