Regards,
Robert Lopez
Hazardous Materials Specialist II
San Joaquin County
Environmental Health Department
1868 E. Hazelton Ave.
Stockton, CA 95205
Work: (209) 953-6214
Cell: (209)815-3793
Fax: (209)468-3433
From: Robert Lopez [EH]
Sent:Thursday, October 18, 2018 2:42 PM
To: info@diamondiceinc.com
Subject: FW: Diamond Ice HQ Inspection Report and Photos
Mr. Parrino,
I have reviewed the latest submittal and have found the following issues:
• Violation#5 - Hazardous Material Inventory:
- Propane, the average amount, maximum amount, and site of the largest container are grossly
under reported at 100 cubic feet;the storage pressure (above ambient) has not been
completed.
- Diesel has not properly reported.
• Violation#6 -The site map is not complete (blank map form attached):
- The site map does not show loading areas and internal roads.A site map shall contain a north
orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and
exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and
storage areas, and emergency response equipment. If a site map element is not applicable for
your facility then list it on the map and label as "NA".
Here are the guidelines for the site map:
1 All required elements need to be addressedon the map. If not applicable, write "NA" or a statement that
says the element is not on site.
2 Multiple maps can be submitted if one map cannot show all of the required elements. If more than one map
is submitted, there has to be one map that shows the overview of the entire facility.
3 All labels, symbols, writing, printing shown on the site map must be legible and oriented in the same
direction.
4 All symbols for required elements must be defined.
5 All acronyms must be defined.
6 Emergency shutoffs need to be labeled as what kind of chemical shutoff(ex. Diesel shutoff), gas, or electric.
7 If the facility has adjacent buildings, label the adjacent buildings.
2
