The work plan included installation of <br /> two additional dual phase extraction ( DPE ) wells and installation , operation and maintenance <br /> of the DPE remediation system . The EHD informed you that on 30 September 2011 , public <br /> notification of pending remediation was posted at your site. The public had thirty days to <br /> express concerns regarding the implementation of the proposed remediation . The EHD <br /> notified AGE on 02 November 2011 that no concerns had been expressed by the public so <br /> the work outlined in the work plan could commence . <br /> In Dual Phase Extraction Well Installation Report, dated 05 January 2012 , AGE <br /> recommended continuation of the remedial installation process but stated that the remedial <br /> system installation was " largely dependent on the availability of funding and pre-invoice <br /> approval of the proposed DPE remedial system from the underground storage tank cleanup <br /> fund . " In Groundwater Monitoring Report — Fourth Quarter 2011 (GWMR), dated 24 February <br /> 2012 , AGE stated that a Final Remediation Plan (FRP) , proposing dual phase remediation <br /> had been submitted to the EHD during the third quarter 2011 and " Implementation of the FRP <br /> is pending a response from the EHD . " <br /> Following the thirty-day public participation period , AGE had been notified by electronic mail <br /> on 02 November 2011 of EHD approval to commence with the work outlined in Remedial <br /> Action Work Plan — August 2011 . Provide the EHD with an update on the installation of the <br /> DPE remediation system by 04 May 2012 . If the system has not been installed , proceed with <br /> the installation of the DPE system immediately. <br />
