If any of the returns contain revenue from a <br /> corporation, partnership, or trust, submit the federal tax returns for that entity for the same three <br /> control years. Refer to the attached chart of required federal tax returns. <br /> In order to qualify for Priority Class C, submit documentation of the number of employees of <br /> your small business (e.g. DE3 form). If you have no employees, submit a signed statement to <br /> verify that fact. <br /> If your claim is accepted, you will not be eligible for reimbursement of cleanup costs associated <br /> with the gasoline USTs removed by ARCO in 1974. The claimant must be a former or current <br /> owner or operator of the leaking tank.
