If any of the returns contain revenue from a <br /> corporation, partnership, or trust, submit the federal tax returns for that entity for the same three <br /> control years. Refer to the attached chart of required federal tax returns. <br /> In order to qualify for Priority Class C, submit documentation of the number of employees of <br /> your small business (e.g. DE3 form). If you have no employees, submit a signed statement to <br /> verify that fact. <br /> If your claim is accepted, you will not be eligible for reimbursement of cleanup costs associated <br /> with the gasoline USTs removed by ARCO in 1974. The claimant must be a former or current <br /> owner or operator of the leaking tank. If your claim is accepted, you will be eligible for <br /> reimbursement of cleanup costs associated with the waste oil tank formerly located on the <br /> southern part of the property. <br /> NOTE: Failure to respond to this request within thirty(30) calendar days from the date of this <br /> letter may result in an ineligibility determination of your claim. <br /> California Environmental Protection Agency <br /> 0 Recycled Paper <br />
