0 • <br /> 3.0 PARTICIPANT QUALIFICATIONS <br /> 3.1 TRAINING REQUIREMENTS <br /> All ERM and subcontractor personnel working at the Nortrax facility will <br /> have completed an extensive training course and have previously worked <br /> at least 3 days at a hazardous-waste site. The training course must be <br /> designed to meet the requirements of 8 CCR, 5192. The training course <br /> must consist of a combination of 40 hours of classroom and field exercises, <br /> plus an annual 8-hour refresher. <br /> All site participants will be required to show proof of current training <br /> prior to field activities. Intended participants without current training <br /> documentation will be barred from site activities. <br /> 3.2 MEDICAL SURVEILLANCE <br /> All on-site personnel, subcontractors, and visitors will be required to have <br /> a written statement from a licensed physician stating they have had a <br /> medical examination which meets the requirements of 8 CCR 5192. This <br /> examination must include pulmonary function testing, as well as <br /> certification by the physician of the employee's ability to wear a negative- <br /> pressure respirator and perform strenuous work. If a person sustains an <br /> injury or contracts an illness related to work onsite that results in lost <br /> work time, he/she must obtain written approval from a physician to <br /> regain access to the site. <br /> 3.3 RECORD KEEPING <br /> Air monitoring via industrial hygiene monitoring or direct reading <br /> instrumentation will become part of the written record. Both medical and <br /> air monitoring data will be retained for 30 years. Training records shall be <br /> maintained in project files and are available for inspection at any time. <br /> Information pertaining to subcontractor training and medical clearance <br /> will also be maintained in project files. <br /> 5164.00-3/4/03 <br /> ERM <br /> 6 <br /> HASP CANON <br />
