(Stantec) is submitting this well destruction work plan as part of the closure package for the ARCO <br /> Service Station No. 213o located at the above referenced address (the "Site", Figures 1 & 2). The Site has <br /> been part of an environmental investigation case under the direction of the San Joaquin County <br /> Environmental Health Department (SJCEHD). This work plan presents a detailed scope of work proposed <br /> for monitoring and remediation well destruction as required by SJCEHD letter dated August 23, 2013 <br /> (Attachment A). Well construction details and the proposed well destruction method for each well are <br /> included in Table 1. <br /> Site Health and Safety Plan <br /> As required by the Occupational Health and Safety Administration (OSHA) "Hazardous Waste Operations <br /> and Emergency Response" guidelines (29 CFR 1910.12o) and by California Occupational Health and Safety <br /> Administration (Cal-OSHA) "Hazardous Waste Operations and Emergency Response" guidelines (CCR <br /> Title 8, Section 5192), the contractor will prepare a site specific Health and Safety Plan (HASP). At a <br /> minimum, the HASP will define the proposed subsurface investigation activities, describe physical and <br /> chemical hazards that may be associated with the work, provide a map to the nearest emergency medical <br /> facility, and include material safety data sheets for any hazardous chemicals that will be used or produced <br /> during the work. A copy of the HASP will be available onsite at all times during field work. The field staff <br /> and contractors performing field activities will review the HASP prior to beginning field operations, as <br /> described in this work plan, at the site. <br /> Permitting <br /> Prior to beginning field work, all necessary permits will be obtained from the SJCEHD and the City of <br /> Stockton including an encroachment permit for the 5 monitoring wells (MW-6, MW-7, MW-14, MW-19, <br /> and MW-21) located in the right-of-ways. <br /> Underground Utility Location and Clearance. Well locations will be marked or cleared of <br /> underground facilities prior to well destruction activities. As required by the California Government Code <br /> 4216, Underground Service Alert (USA) North and 811 will be notified at least two working days before <br /> drilling activities. Well locations will be marked with white paint to indicate the area of drilling. This will <br /> allow USA North members, including local utility companies, to identify and mark the horizontal path of <br /> Design with community in mind <br />
