EI Dorado well destruction work plan <br /> Good afternoon, <br /> This email serves the following purposes related to the subject site: <br /> 1) Update request on well destructions <br /> 2) Address change of permit fees <br /> 3) Outline permit requirements <br /> 4) Specify proper waste management during destruction activities <br /> The San Joaquin County Environmental Health Department ( EHD) has not yet received an update regarding the status of <br /> the permit application or plans to destroy monitoring wells pertaining to the subject site . Please provide an update. <br /> The recently .implemented EHD well fees applying to the subject site are the following: Per parcel, an initial fee of $ 130 <br /> per monitoring well (maximum $650) will be required for permit issuance. Additional time spent by this department's <br /> staff pertaining to activities at the subject site beyond the initial paid fee will be billed at $130 per hour. Please submit <br /> the following items to my attention for permit issuance: <br /> • Two EHD Well and Boring Permit application forms (one for each parcel/right-of-way) <br /> • City of Stockton Encroachment Permit <br /> • Licensed Contractor's Declaration form (LCD) <br /> • Master File Record form ( MFR) <br /> • Fee of $1,300. (Site parcel: $ 130 x 5 = $ 650. Stockton right-of-way: $ 130 x 5 = $650. $650 x 2 = $ 1,300) <br /> The EHD will contact Stantec when the permit is issued .
