The following is an itemized list of aboveground petroleum storage act violations that <br /> have not been addressed for J & L MARKET as of December 26, 2019. <br /> Open violations from July 31, 2019 inspection <br /> Violation#618-Failed to keep records of procedures, inspections,or integrity tests for three years. <br /> Copies of inspection and testing records were not found on site. Inspections and tests must be conducted in <br /> accordance with the written procedures developed in the Spill Prevention, Control, and Countermeasure(SPCC) <br /> Plan. The SPCC plan references SP-001 standards. Records of these inspections and tests must be signed by the <br /> appropriate supervisor or inspector and kept on site with the SPCC Plan for a period of three years. Immediately <br /> locate a copy of all inspection and testing records for the last three years, maintain them on site, and submit copies <br /> to the EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#706-Failed to provide and maintain adequate secondary containment. <br /> The 10,000 gallon diesel tank was observed with insufficient secondary containment. Per a Professional Engineer <br /> certified secondary containment calculations dated 08/09/2011 the secondary containment for the 10,000 tank only <br /> holds 6,942 gallons.This does not provide secondary containment for the entire capacity of the tank and sufficient <br /> freeboard to contain precipitation. All bulk storage tanks must be provided with a secondary means of containment <br /> for the entire capacity of the tank and sufficient freeboard to contain precipitation. Immediately provide sufficient <br /> secondary containment for this and all other tanks at this facility. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#712-Failed to test or inspect each container for integrity based on industry standards. <br /> The SPCC plan references the STI-SP001 industry standard. The age of the tank could not be determined and a <br /> certified STI SP-001 inspection report could not be located. Each aboveground container shall be tested and <br /> inspected for integrity on a regular schedule and whenever repairs are made. The qualifications of personnel <br /> performing tests and inspections, frequency and type of testing and inspections that take into account container <br /> size, configuration, and design shall be determined in accordance with industry standards. Examples of these <br /> integrity tests include, but are not limited to: visual inspection, hydrostatic testing, radiographic testing, ultrasonic <br /> testing, acoustic emissions testing, or other systems of non-destructive testing. Comparison records and other <br /> records of inspections and tests must be maintained on site. Immediately conduct the necessary testing and submit <br /> a copy of the test results to the EHD, or provide equivalence as allowed by CFR 112.7(a)(2). <br /> Page 1 of 2 <br />
