Copies of inspection and testing records were not found on site. Inspections and tests must be conducted in accordance with the written procedures developed in the Spill Prevention,Control, and Countermeasure(SPCC)Plan. The SPCC plan references SP-001 standards. Records of these inspections and tests must be signed by the appropriate supervisor or inspector and kept on site with the SPCC Plan for a period of three years. Immediately locate a copy of all inspection and testing records for the last three years,maintain them on site, and submit copies to the EH D. This is a repeat violation,Class If. 706 CFR 112.8(cx2)Failed to provide and maintain adequate secondary containment. The 10,000 gallon diesel tank was observed with insufficient secondary containment. Per a Professional Engineer certified secondary containment calculations dated 08/09/2011 the secondary containment for the 10,000 tank only holds 6,942 gallons.This does not provide secondary containment for the entire capacity of the tank and sufficient freeboard to contain precipitation. All bulk storage tanks must be provided with a secondary means of containment for the entire capacity of the tank and sufficient freeboard to contain precipitation. Immediately provide sufficient secondary containment for this and all other tanks at this facility. This is a repeat violation,Class If. 712 CFR 112.8(cx6)Failed to test or inspect each container for integrity based on industry standards. The SPCC plan references the STI-SPD01 industry standard.The age of the tank could not be determined and a certified STI SP-001 inspection report could not be located. Each aboveground container shall be tested and inspected for integrity on a regular schedule and whenever repairs are made. The qualifications of personnel performing tests and inspections,frequency and type of testing and inspections that take into account container size, configuration,and design shall be determined in accordance with industry standards. Examples of these integrity tests include,but are not limited to:visual inspection,hydrostatic testing, radiographic testing, ultrasonic testing, acoustic emissions testing,or other systems of non-destructive testing. Comparison records and other records of inspections and tests must be maintained on site. Immediately conduct the necessary testing and submit a copy of the test results to the EHD,or provide equivalence as allowed by CFR 112.7(a)(2). This is a repeat violation,Class If.
