Submit a list of all the record fee <br /> title owners with their mailing addresses to PHS/EHD so they can be notified of <br /> our intent to issue a closure letter. If you are the sole property owner, provide a <br /> statement of that fact to PHS/EHD, to fulfil this requirement.
