The work has not occurred and the 90-day condition date has been exceeded. <br /> Remedial action should proceed as outlined in the approved work plan(also refer-to the <br /> previously submitted AGE Corrective Action Plan dated May 19%and the addendum to this plan <br /> dated October 14, 1997). The work outlined in the above referenced work plant should be <br /> initiated by September 1,1999. If the work is not initiated by September 1, 1999 the site will <br /> not be in compliance with PHS/EHD directives and reimbursement for corrective action costs <br /> from the State Water Resources Control Board Cleanup Fund may be jeopardized. <br /> PHS/EHD was informed by AGE on July 22, 1999 that there would be changes in the above <br /> referenced work plan. PHS/EHD received a fax from AGE an July 22, 1999 requesting an <br /> extension for work initiation until August 23, 1999. Any changes in the above referenced work <br /> plan should be submitted to PHS/EHD for approval prior to work start date.
