This waste is being disposed of as <br />non -hazardous waste (last disposal occurred on 12-4-2015). This waste was historically hauled away on a <br />hazardous waste manifest on 2-13-13 and 9-13-06. A generator shall make a hazardous waste determination and <br />keep a record of any test results, waste analyses, or other determinations made in accordance with hazardous <br />waste regulations for at least three years from the date that the waste was last disposed of. Immediately locate a <br />copy of the waste analysis conducted for the oil waster separator waste and submit a copy to the EHD. If a copy is <br />unavailable, immediately make a hazardous waste determination and begin managing the oil water separator waste <br />under Title 22 hazardous waste regulations. <br />This is a Class II violation. <br />108 <br />CCR 66265.53 Failed to maintain a hazardous waste contingency plan onsite and submit to required agencies. <br />The contingency plan was not available at the time of inspection. A copy of the contingency plan and all revisions to <br />the plan shall be maintained at the facility at all times. Immediately locate a copy of the contingency plan for the <br />facility and submit a copy of the plan to the EHD. <br />This is a Class II violation. <br />111 <br />CCR 66265.16(d) Failed to maintain complete personnel training records. <br />Employee training records for the management of hazardous waste were incomplete; items 1, 2, and 3 (detailed <br />below) were not available for review. <br />Employee training records shall include: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to <br />each person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility.
