Contact the fire department and San Joaquin Valley Air District for any applicable <br />permits. <br />2. Any changes shall be submitted to Environmental Health Department (EHD) for approval <br />prior to commencing work. All new addendums supercede the previous ones. <br />3. Any piping or components replaced must be approved be EHD and have an independent <br />third party approval. <br />4. At least one person who is certified as an ICC UST Installer/Retrofitter must be onsite at <br />all times when work is performed on any portion of the UST system and have <br />manufacturer's certification on components being repaired/replaced. <br />5. Everyone who works on components must have manufacturer's certification on that <br />component. <br />6. Contact EHD for inspection of the excavated area to determine the extent of the damage. <br />A line test may be required. <br />7. If concrete is broken near the underground storage tanks they must be have an integrity <br />test performed once concrete is poured in accordance with manufacturer's guidelines. <br />Results must be received by EHD within 7 days of returning the site to operating <br />condition. <br />8. This permit expires on December 26, 2006. <br />9.
