SANJ O A Q U I ` ,I�,II Environmental Health Department <br /> COUNTY <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> r&j dondero inc 20480 E Copperopolis Rd, Linden August 06,2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> Three 156 cu ft Oxygen cylinders and two 145 cu ft Acetylene cylinders were observed in the shop that was not <br /> reported in the hazardous materials inventory.Any material that meets or exceeds the reportable quantity shall be <br /> reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> Note: Three 316 cu ft cylinders of 75%Argon, 25%Carbon Dioxide mixture were observed in the shop.The reporting <br /> quantity for simple asphyxiants is 1,000 cu ft. If this material is stored or handled on site at any time in the year at <br /> reportable quantities,the inventory must be updated to include this material. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The site map did not address loading areas, access and entry points, emergency shut-offs, or emergency response <br /> equipment.A water run-off collection pool was observed on site that is not labeled on the map.Additionally, three 55 <br /> gallon propane tanks were observed in the shop were not on the site map and location was not identified in the <br /> inventory.A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then <br /> list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were available for review as a part of the Injury Illness Prevention Plan on site; <br /> however,the Emergency Plan procedures submitted to CERS was missing the following information: includes <br /> hazardous waste procedures, internal response procedure,facility evacuation procedures. A business plan shall <br /> include the following emergency response procedures for a release or threatened release of hazardous materials, <br /> including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0020209 PR0540920 SCO01 08/06/2019 <br /> EHD 19-01 Rev.09/27/2018 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
