An extension was granted to May 8, 2004. As of this date, we <br />have not received any additional documentation. Please refer to our letter of March 8, 2004 for any <br />documents referred to below (which are not being duplicated with this letter). <br />After reviewing your claim application to the Cleanup Fund, we find that the following additional <br />information is still needed to determine your eligibility for placement on the Priority. List: <br />Please clarify correct site address. Page two of your claim application identifies site as Former Tracy <br />Diner, 95 West 11"' Street in Tracy. Below are two sites listed on Geo Tracker. Are these the same <br />site? <br />L Former Restaurant @ 95 110' St W., Tracy <br />2. Panetta Property @ 95 IE. 1 lth Street <br />• Page one (1) of your claim application was incomplete. Please complete the section for Tax <br />Identification No. Claimant's tax identification number must correspond with the claimant name <br />(e.g. individual -social security number; corporation, partnership, estate, trust -federal tax identification. <br />number). <br />• Page two (2) of your claim application doesn't reflect information you submitted. Please revise or <br />explain. Page 2 lists: <br />1. Date of Release Discovered 05/10/00 (prior to claimant's; acquisition of site). The first <br />UST Unauthorized Release (LEAK) / Contamination Site Report (dated 05110/00) <br />identifies the Responsible Party as JLN Farms Inc. et.al. It appears that there was another <br />release discovered. The second LEAK Report (dated 10/21/03) identifies Joe Panetta as <br />the Responsible Party.
