Fees shall be paid once a year at the time <br />of submittal of an inventory form. Immediately pay the annual fee to the EHD Accounting Office located at 1868 E <br />Hazelton Avenue, Stockton, CA 95205. Fee may also be paid online at www.sjcehd.com (Convenience fee may <br />apply). <br />This is a repeat violation, Class II. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br />The Hazardous Materials Inventory forms for all chemicals were not complete. Obsolete federal hazard categories <br />were listed on inventory. This information must be current and submitted as part of the business plan. This violation <br />was corrected on site. <br />This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />The site map was not complete as part of the business plan. The site map is missing utilities shut offs and a spill kit <br />locations. Please add the missing elements to the site map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as “NA". This violation was corrected on site. <br />This is a Class II violation. <br /> 8 HSC 25505(a)(4) Failed to submit a training program or program is not reasonable or adequate for facility. <br />The training program said provided in the facility information section. Please change to say provided in the <br />emergency response and contingency plan section. <br /> Training programs for all employees and annual training, including refresher courses, shall include safety <br />procedures for the event of a release or threatened release of a hazardous material, including, but not limited to, <br />familiarity with the facility's emergency response plans and procedures. These training programs may take into <br />consideration of each employee.
