i <br /> Independent Shell-Branded Service Station Page 2 of 2 <br /> 4315 Waterloo Road July 29, 2014 <br /> Stockton, CA 95215 <br /> • Following pressure grouting, enlarge the top three feet of the borehole at least one foot <br /> larger in diameter than the diameter of the auger originally utilized to install the well; cut <br /> off and remove the casing six inches above the bottom of the excavation before inserting <br /> the grout as described in the workplan; <br /> • Notify the EHD at least two weeks prior to implementing fieldwork activities so that an <br /> inspector can schedule to be present for the well destructions; and <br /> • Include the signed and dated manifests of soil and groundwater waste removal when <br /> submitting the final well destruction report. <br /> During the public participation period, which ended on 27 May 2014, EHD received no <br /> comments or concerns from interested individuals and agencies regarding the pending closure <br /> of this site. You may proceed with the well destructions if in compliance with the conditions <br /> noted above. <br /> Please submit an EHD Well and Boring Permit Application form along with a Master File Record <br /> form, an access agreement from the adjacent property owner where an off-site well is located, <br /> and appropriate fees for each parcel. The permit fee for the destruction of each well is $125, <br /> along with a $375 Inspection Activity fee per parcel. A fee of $2,000.00 (10 x $125 + 2 x $375) <br /> will be required for the well destruction permits. (Please note that on 01'August 2014 the EHD <br /> fee rate per well will increase to $130.
