Less than 1/4 cup of fuel was observed insite the sump. The technician onsite cleaned the fuel <br />with absorbent pad and placed it into the onsite hazardous waste drum. All primary containment must be <br />impervious to the liquid and vapor of the substance contained. Immediately discontinue use of failed <br />piping and have a properly licensed, trained, and certified contractor replace or repair the failed piping <br />under permit and inspection of the EHD. If the failed component can't be replaced immediately, there is <br />a possibility that the 87 -octane UST system may be red tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: <br />Title: <br />Elena Manzo <br />i <br />rN 7EHD 23-02-003 Rev 04119/12 VCONTINUATION FORM <br />
