The following is an itemized list of underground storage tank violations that have not <br />been addressed for WATERLOO FOODMART as of May02, 2018. <br />Open violations from April 10, 2018 inspection <br />Violation #101 - Failure to obtain a valid permit to operate from the EHD including but not limited to unpaid <br />fees. <br />A permit to operate the UST system has not been issued for 2018. No person may own or operate an UST unless a <br />permit for its operation has been issued by the local agency to the owner or operator of the UST system. <br />Immediately obtain a permit to operate a UST system from the EHD. An "Operating without a Permit' penalty <br />applies. <br />Violation #107 - Plot plan/site map not submitted or failed to completely show where monitoring is <br />performed. <br />An accurate UST Monitoring Site Plan was not submitted. The site map is missing piping for the vent sump, and is <br />missing locations of the vacuum sensors. A site plan must be submitted identifying the locations where monitoring <br />will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />Violation #111 - Failed to test monitoring and leak detection equipment annually. <br />Annual monitoring system certification and leak detector testing was not completed on 4/10/18 and was due by April <br />30, 2018. Brine communication testing on the 87 -product STP sump was required to be completed within the month <br />of the monitoring system certification. Testing has not been completed and is past due. These tests are required <br />once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be <br />aware that a follow up inspection will be charged at our current hourly rate. <br />Annual monitoring system certification and leak detector testing was not completed on 4/10/17 and was due by April <br />30, 2017. Vacuum and brine communication testing was required to be completed within the month of the <br />monitoring system certification. Testing has not been completed and is 11 months past due. These tests are <br />required once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. <br />Please be aware that a follow up inspection will be charged at our current hourly rate. <br />Violation #121 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on 4/10/17 and an inspection report was issued identifying information to be submitted <br />to bring this site into compliance. This information was required to be submitted by 5/10/17. This information has <br />not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report <br />shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or <br />proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions <br />taken or proposed is submitted within 30 days of receiving an inspection report. The open violation for the facility <br />includes: HSC 25290.1(e) VPH monitoring in the interstitial space not maintained. Communication in the 87 and 91 <br />product submersible turbine pump (STP) and fill sumps of the vacuum pressure hydrostatic (VPH) system were not <br />tested during the inspection on 4/10/17. The interstitial space of the underground storage tank system shall be <br />maintained as designed under constant vacuum such that a breach in the primary or secondary containment is <br />detected before the hazardous substance is released into the environment. Immediately contact a properly licensed, <br />trained and certified contractor to install and test communication of the VPH system components under permit and <br />inspection by the environmental health department. <br />Page 1 of 4 <br />
