101: failure to obtain a valid permit to operate from the Ehd. <br />There was a whole misunderstanding and a miss communication on that with the previous inspector. <br />She told us that we needed a special part placed in (comm port)? 1 guess you call it. We had that <br />placed for future testing by Elite four Never a requirement before with all these years, never the less <br />had it installed by elite four. Have invoice for that. However we did do the necessary testing already. <br />We immediately tested site I have also paid, but no one notified that we needed this test. When 1 <br />called, I was told we are short staffed and can't mail letters out for everything". In good faith my <br />permit money was sent out, and I never received anything stating "we have your payment, but you need <br />this..... done " So we have completed our testing and hope our permit will be mailed out. Never our <br />intention to not "comply", but .somehow this particular year the inspector was things done a whole <br />different way with this "part " put in. <br />107 Plot map was done by Afford a test to best of our knowledge and uploaded on Cers on 3-23-18. <br />not sure why that was not looked at. I will attach of the copy that was uploaded and there is a copy at <br />the site. <br />111 This was completed with Aaron on 4-17-18. with results sent in to Ehd 4-24-18. <br />121 There was a response faxed last year to the inspector. lam not sure why there was no letter sent <br />stating nothing received and waited one full year for this to arise. It has never happened in the past <br />that we do not send Rtc back. We are always sending RTC back and understand the importance of it. <br />She was training with people that day and 1 am not sure who received or where it went. She It was <br />very time consuming and a lot of confusion that day because she had two people training with her and <br />the tech can vouch for that as well. This somehow has fell through the cracks with no communication., <br />Never the less have had the tests done. <br />201 Failure to maintain monitoring, we have had so many problems with this site that we decided to <br />put brand new equipment in. Paying so much for service calls and it being ok for days and then <br />alarms again. for all alarms we noted, we did call elite four to come check. We have several invoices <br />for this as well. Loan was approved and equipment had been ordered, but never knew that it would be <br />back ordered. So again, this all being changed out and we have noted all alarms and were waiting and <br />hoping and pleading our equipment would arrive. This is being done with the permit that Elite four is <br />getting. We are replacing udc's and all repairs to sumps. Hopefully no more alarms and issues. We <br />have and will continue to write all alarms down and will let Alex know as well. <br />304 Elite four will be addressing this issue as well when they start work on the sumps and udc's and <br />making sure everything is addressed properly per requirements. <br />314 We are getting all this fixed under the permit Elite four is pulling. Elite four recommended that we <br />get everything replaced because of issues and to fix it would probably require everything new anyways. <br />Also stated it wouldn't be feasible to keep putting brine in. <br />I hope I have addressed all violations properly and correctly and to the best of my knowledge. Again <br />we very soon are having all this changed out. agglts was the loan process and wait period for all <br />equipment since this site is different and require different parts. We have constantly been bugging the <br />vendors. Please let me know ifI have addressed all this properly and correctly. 1 will also be emailing <br />
