The report is to show the: <br /> -status of investigation and cleanup activities including the results of all investigations <br /> completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion of the <br /> investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and any contaminated <br /> soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if known, and changes in the above <br /> measurements over the last quarter. <br /> These current quarterly reports are now past due. Please submit them to PHS-EHD and CVRWQCB <br /> immediately.
