Tom Piskor <br /> RE: Tank(s) Removal @ <br /> 4315 Waterloo Road (Shell) <br /> The permit application to remove 2 underground storage tanks from the above referenced <br /> facility has been approved with the following conditions: <br /> e,X All analytical results must come directly from a third party California certified <br /> laboratory with affixed chain of custody or this office will not accept them. <br /> Z ai All hazardous waste must be treated, stored or disposed of at a permitted facility. <br /> ,3*' See pages 7 & 8 of the permit application for required analytical tests. Note: Your <br /> plan states that all samples will be taken from the ends of the tank, but page 7 of the <br /> permit application states that all samples shall be taken evenly spaced along the tank. <br /> \-4. A copy of the fire department permit must be submitted at the time of removal. <br /> 6 5. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> ,zyAnalytical results,mailed from the lab <br /> (b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br /> (c) Tank Closure Certification Form(DTSC form 1249) <br /> (d) Hazardous Waste Manifest for piping,rinsate,residual fuel, or waste oil receipt <br /> 6. The numbers of samples to be taken will be in accordance to the tank size as per page <br /> 7 of this permit. <br /> �1 9.
