Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />The permit application to remove 2 underground storage tanks from the above referenced <br />facility has been approved with the following conditions: <br />1. All analytical results must come directly from a third party California certified <br />laboratory with affixed chain of custody or this office will not accept them. <br />2. All hazardous waste must be treated, stored or disposed of at a permitted facility. <br />3. See pages 7 & 8 of the permit application for required analytical tests. Note: Your <br />Dlan states that all samples will be taken from the ends of the tank, but pane 7 of the <br />permit application states that all samples shall be taken evenly spaced along the tank. <br />4. A copy of the fire department permit must be submitted at the time of removal. <br />5. In addition, the following information must be submitted within 30 days of the <br />removal date: <br />(a) Analytical results, mailed from the lab <br />(b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br />(c) Tank Closure Certification Form (DTSC form 1249) <br />(d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt <br />6. This permit expires on December 20, 2004. <br />7. This site shall be sampled for unleaded fuel. <br />8. The numbers of samples to be taken will be in accordance to the tank size as per page <br />7 of this permit. <br />9.
