There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone:the name and phone number of the emergency coordinator, location of fire <br /> extinguishers,spill control equipment,and if present,fire alarm, and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> A copy of uniform manifest 013482539JJK for the disposal of hazardous waste for 2015 were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit a <br /> copy of the missing manifest to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 2/17/18. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br /> information online to the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov.
