Health and Safety Plan <br /> 455 E lith St <br /> Tracy,California <br /> 3.3 Physical Hazards <br /> Physical hazards discussed under this section include underground and overhead utilities, drill <br /> rigs, machinery and moving parts, electrical hazards, noise,slips,trips,and falls, motor vehicle <br /> and heavy equipment operation, and ergonomic hazards. <br /> 3.3.1 Underground and Overhead Utilities <br /> Reasonable efforts will be made to identify the location(s)of underground utilities (e.g., pipes, <br /> electrical conductors,fuel lines, and water and sewer lines) before mechanized soil intrusive <br /> work is performed.The state underground utility notification authority Underground Services <br /> Alert (USA),will be notified at least two business days (not including the day of notification) <br /> prior to starting field activities. USA in turn will notify representatives of the utility companies <br /> who will mark the location of underground services entering the property. USA markings will be <br /> noted prior to drilling to prevent damage to utility lines. <br /> The Project Manager is responsible to ensure that underground utility locations are identified <br /> prior to the commencement of any subsurface (> 1 foot) activities. Resources include site plans, <br /> utility companies, and regional utility locating services.The proper utility company personnel <br /> should certify the deactivation of utilities, and the certification should be retained in the <br /> permanent log. <br /> In accordance with CCR,Title 8,Section 1541(Excavations), an excavation shall not commence <br /> until: <br /> • The excavation area has been marked as specified in Government Code Section 4216.2 <br /> by the excavator; and <br /> • The excavator has received a positive response from all known owner/operators of <br /> subsurface installations within the boundaries of the proposed project;those responses <br /> confirm that the owner/operators have located their installations, and those responses <br /> either advise the excavator of those locations or advise the excavator that the <br /> owner/operator does not operate a subsurface installation that would be affected by <br /> the proposed excavation. <br /> Only qualified persons shall perform subsurface installation locating activities, and all such <br /> activities shall be performed in accordance with this section and Government Code Sections <br /> 4216 through 4216.9 <br /> When excavation or boring operations approach the approximate location of subsurface <br /> installations,the exact location of the installations shall be determined by safe and acceptable <br /> means that will prevent damage to the subsurface installation, as provided by Government Code <br /> Section 4216.4. <br /> Appendices Terraphase Engineering Inc. <br />
