Health and Safety Plan <br /> 455 E lith St <br /> Tracy,California <br /> 7.0 ACCIDENT REPORTING <br /> Workers are required to immediately report all accidents, incidents, near-miss incidents, <br /> injuries, illnesses and injuries requiring first-aid (no matter how trivial)to their supervisor <br /> and/or the SSHO. <br /> In the event of a worker injury or illness,the SSHO must be notified,who will in turn <br /> immediately report the incident to the Project Manager to assist with the coordination of <br /> required medical assistance and related workers' compensation case management follow-up. <br /> Should an incident such as a serious injury(requiring hospitalization), explosion,fire, or a spill or <br /> release of toxic materials occur, during the project,the SSHO will immediately report the <br /> incident to the project manager and appropriate government agencies.The written report must <br /> include the following information: <br /> • Name, organization,telephone number, and location of the contractor <br /> • Name and title of the person(s) reporting the incident <br /> • Date and time of the incident <br /> • Location of the incident <br /> • Approximate chronological summary of details occurring before and at the time of the <br /> incident <br /> • Cause of incident(if immediately known) <br /> • Casualty information (fatalities or disabling injuries) <br /> • Details of any existing chemical hazard or contamination <br /> • Estimated property damage (if applicable) <br /> • Nature of the damage and effect on the contract <br /> • Actions taken to preserve safety and security <br /> • Other damages or injuries sustained <br /> • Witness statements <br /> If a utility strike does occur, report this to the Project Manager and utility operator as soon as it <br /> is safe to do so.The contact info for potential utility operators near the Site are available in the <br /> USA ticket which is included in Appendix E. <br /> Terraphase Engineering Inc.
