Eleventh St. <br /> Tracy, CA 95376 <br /> San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) has reviewed the "Quarterly Groundwater Sampling <br /> Report" and the "Site Characterization Report" dated Nov. 8, 1991 <br /> and submitted by Remediation Risk Management, Inc. . PHS/EHD has <br /> the following comments: <br /> Pursuant to Section 2652 (d) of the Underground Storage Tank <br /> Regulations (California Code of Regulations, Title 23, <br /> Subchapter 16) a quarterly monitoring report shall be <br /> submitted to Public Health Services, Environmental Health <br /> Division and Central Valley Regional Water Quality Control <br /> Board (CVRWQCB) until the investigation and cleanup of the <br /> site is deemed adequate. The report is to show the: <br /> -status of investigation and cleanup activities including <br /> the results for all investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for <br /> the completion of the investigation of the site and <br /> remediation; <br /> -method and location of disposal of the released <br /> hazardous substance and any contaminated soil, <br /> groundwater or surface water; <br /> -manifest required for transport for hazardous substances <br /> -monitoring well data including groundwater elevation, <br /> ph, conductivity, gradient; <br /> -analytical results for groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if <br /> known, and changes in the above measurements over the <br /> last quarter. <br /> Please give PHS/EHD advance notice 48 hours prior to the sampling. <br /> In order to obtain a "zero line" of contamination, additional <br /> monitoring wells will need to be constructed. PHS/EHD requests a <br /> workplan for additional monitoring wells to define the extent of <br /> contamination by February 23, 1994. <br /> A Division of San Joaquin Cowry Health Care Semces �� <br />
