Before a <br /> NFAR letter can be provided to you,the monitoring wells at the''site must be properly destroyed. <br /> Submit a work plan to destroy the wells, an EHD permit application,and$60 permit fee. Please note-pplica <br /> - <br /> that monitoring wells in areas of contamination may only be des4royed by removing all of the well <br /> contents by drilling out the original borehole and filling it with grout. <br /> In addition to the above,please provide EHD certification in writing that all record fee title owners <br /> have been notified of your request for closure. This is in accordance with Section 25297.15 of the <br /> California Health and Safety Code. Submit a list of all record f6e title owners and their mailing <br /> addresses. EHD will send notification to each title owner of the:� <br /> pending closure letter. If you are the <br /> sole property owner,provide a statement to this effect to EHD.
