GROUND ZERO ANALYSIS, INC. <br /> 1714 Maul street <br /> Escalon,California 95320-1927 <br /> Telephone (249)838-9888 <br /> Facstinile (209)838-9883 <br /> ID <br /> September 21, 2004 <br /> SEP 2 2 2004 <br /> ENVIRONi Lit I IiLALI h <br /> Ms Vicki McCartney FERMI USERVICES <br /> San Joaquin County Environmental Health Department <br /> 304 East Weber Avenue, Third Floor <br /> Stockton, CA 95202 <br /> Subject Summary Report for the Destruction of Wells MW1 through MW3 <br /> 612 West Eleventh Street, Tracy, CA <br /> Dear Ms McCartney <br /> On September 7, 2004 Ground Zero Analysis, Inc (Ground Zero) supervised the destruction of <br /> groundwater monitoring wells MW1 through MW3 at the subject site The work was done in <br /> response to the California Regional Water Quality Control Board letter requesting site closure dated <br /> August 2, 2004 The site is located at 612 West Eleventh Street in Tracy, California, and its <br /> location is shown on Figure 1 <br /> SITE ACTIVITIES <br /> V&W Drilling (C57 License #720904) destroyed wells MW 1 through MW3 under the supervision <br /> of a Ground Zero geologist Monitoring wells MWl, MW2 and MW3 were destroyed by <br /> overdrilling with 8-inch diameter hollow stem augers to a total depth of 27 feet The borings were <br /> then backfilled to the surface with neat cement grout, emplaced at depth through the augers <br /> Approximately 210 gallons of neat cement grout were used to fill the three well bore holes Figure <br /> 2 shows the locations of the site wells Attachment A contains copies of well logs <br /> All of the drill cuttings produced were placed in four 55-gallon drums and were removed from the <br /> site by V&W Drilling The four drums are temporarily being stored at V&W's equipment yard in <br /> Rio Vista, California The drummed waste will be transported under non-hazardous manifest to <br /> Aqua Clear Farms, a Class II waste disposal facility in Rio Vista Six empty 55-gallon drums remain <br /> in the southwest corner of the site, adjacent to the retention pond Their removal is being promptly <br /> scheduled The empty drums will be transported to Ground Zero's storage yard in Escalon, California, <br /> and will be re-used by Ground Zero at various non-hazardous waste sites <br /> i <br /> G IGROUNDZEISouzalReponslWellDestructionRepot doc <br />
