(Horizon) has <br /> prepared this Well Abandonment Letter Report for the above-referenced site (see attached <br /> Figure 1). The purpose of this work was to properly abandon four groundwater monitoring <br /> wells at the locations shown on the attached Site Plan (Figure 2). This work was approved <br /> by the San Joaquin County Public Health Services/Environmental Health Division (SIC <br /> PHS/EHD) and concurred by the California Regional Water Quality Control Board, Central <br /> Valley Region(CRWQCB, CVR) in their letter dated January 28, 1999, as part of regulatory <br /> case closure completion (copies in Attachment A). <br /> Field Work <br /> The work included: obtaining the necessary well abandonment permits, abandoning four <br /> groundwater monitoring wells (MW-2 through MW-5) by overdrilling, and prepanng this <br /> report. All activities were completed in compliance with Bulletin 74-90 of the Cali ornia <br /> Well Standards and the San Joaquin County Public Health Services/Environmental Health <br /> Division(SIC PHS/EHD) guidelines. All field work foilowed Horizon's Field Methods and <br /> Procedures (copy in Attachment B) and the previously submitted Letter Work Plan for Well <br /> Abandonment(Horizon, February 10, 1999). <br /> Horizon obtained the well abandonment permit prior to beginning the work (copy in <br /> Attachment C). A site-specific Health & Safety Plan was prepared, and Spectrum <br /> Exploration, Inc.
