GTI subsequently submitted the required items in our "Site Closure <br /> Checklist" correspondence dated August 31, 1998. <br /> • In a September 16, 1999 letter responding to our site closure request, Mr. Ronald Rowe, <br /> Sr. EHS of the SJC PHS/EHD, directed the completion of a 2000 ft well receptor survey <br /> and a work plan to evaluate the risk to the existing on site domestic well. <br /> • GTI submitted the required items in our "Work Plan for Additional Site Characterization <br /> & Site Closure" dated December 22, 1999, and our "Revised Work Plan -Additional Site <br /> Characterization & Site Closure" dated March 15, 2000. The revised work plan included <br /> the following tasks: <br /> 1. A comprehensive well survey within a 2,000-foot radius of the former UST. <br /> 2. Perform a video survey of the old domestic well to evaluate the construction of the <br /> well and the condition of the casing prior to abandonment. <br /> 3. Abandon the old domestic well and the eight monitoring wells. <br /> 4. Install a new domestic well and connect it to the water piping system. <br /> • In an April 11, 2000 letter, Ms. Margaret Lagorio, Sr. EHS of the SJC PHS/EHD, <br /> approved our March 15, 2000 revised work plan. Ms. Lagorio also stated that the <br /> destruction of the monitoring wells should not proceed until the CVRWQCB has <br /> concurred with the proposed site closure request. <br /> • In a September 6, 2000, telephone conversation, Ms. Margaret Lagorio, Sr. EHS of the <br /> SJC PHS/EHD, informed GTI that Mr. Marty Hartzell of the CVRWQCB required that <br /> the wells be sampled one more time with additional analyses for gasoline oxygenates. <br /> $ With the submittal of this report, C & B Equipment has complied with the regulatory <br /> directives issued by the SJC PHS/EHD and CVRWQCB: <br /> 1. The monitoring wells and "old" domestic were sampled on August 31, 2000 and analyzed <br /> for gasoline oxygenates in accordance with the CVRWQCB directive. <br /> 2. The new domestic well was completed on October 9, 2000 and is currently supplying the <br /> facility with water. <br /> 3. The 2000 ft well receptor survey was completed and a report submitted on August 10, <br /> 2000. <br /> 4. The "old" domestic well was video logged on June 26, 2000. It's destruction costs have <br /> been approved by the USTCFP and it will be destroyed by Calwater Drilling Company <br /> r (to be completed by November 30, 2000, according to Calwater's scheduling <br /> availability). <br /> The C & B Equipment property is in escrow and the new owner's lender is awaiting closure <br /> certification prior to granting funding for the transaction. On behalf of C & B Equipment, <br /> GTI suggests that the corrective action requirements for the investigation and mitigation of <br /> UST release sites have been met and we hereby requests site closure to expedite the property <br /> j transfer process. <br /> 's � <br />
