The Maintenance Agreement shall <br /> remain in force for the duration of operation under this permit.A video showing the condition of all <br /> County maintained roads along the planned haul route to State Route 4 shall be submitted to the ! <br /> Department of Public Works and shall be reviewed by the Department prior to the start of excavation.As <br /> part of the Maintenance Agreement,a cash deposit ar security,adjusted every five(5)years, in the <br /> amount and form acceptable to the Department of Public Works shall be filed with(lie Department with a <br /> guarantee that if the permittee fails,refuses or neglects to repair,under permit,any damages to any <br /> County roads along the haul route which may result from permittec's operation,tine Department of Public <br /> Works may do the work necessary to restore County roads along tite haul route to equal to or better than, <br /> original condition,using the fronds from said cash deposit or security.The fee for the Maintenance <br /> Agreement will be based on the cut-rent schedule at the time of payment. Applicant shall submit a"Stornn Water Pollution Prevention Plan"(SWPPP)to the Department of Public <br /> Works for review.A SWPPP preparation guide is available at the Department of Public Works.A copy of <br /> the SWPPP shall be maintained on the project site and be presented to any County,State or Federal <br /> employee upon request. <br /> 12. The Traffic Impact Mitigation Fee will be required for this development. The fee is due and payable prior <br /> to any work being performed onsite under this permit. The fee shall be autonnatically adjusted July I of <br /> each year by tine Engineering Construction Cost index as published by the Engineering News Record. <br /> Tine fee will be based on the current schedule at the time of payment. The Regional Transportation Impact Fee shall be required for this development.The fee is due and <br /> payable prior to any work being performed onsite sunder this pernnit.
