r <br /> Cardna� <br /> November 3,2014 ATC <br /> Glover Well Destruction Report <br /> Shaping the Future <br /> Cardno ATC requested a variance for the well destruction method regarding these two well and was <br /> approved by SJCEHD. <br /> 2.4 Well Destruction Activities <br /> Cardno contracted with Gregg Drillind_&�Testing, Inc., a C-57 licensed drilling company, to complete the I <br /> well destructions. All five remediation wells and monitoring wells, MW-1 through MW-5, MW-7, and MW- <br /> 8 were abandoned August 25 through August 27, 2014. All twelve wells were destroyed by over drilling <br /> to total depth and then grouting through a tremie pipe from the bottom up. MW-6 and MW-9 were <br /> destroyed on August 29, 2014 by grouting in place and then adding air at 25 psi for five minutes. All well <br /> boxes were removed prior to well destruction activities. Table 1 summarizes the well construction and <br /> destruction detail. Additional cement was poured into the top of the well borehole, as necessary. Field <br /> notes of the destruction activities are in Attachment 2. The surface pavement was repaired to match the <br /> surrounding grade. <br /> As required by the Department of Water Resources, well completion reports were completed, signed by <br /> Gregg Drilling, and submitted to the Department of Water Resources. Copies of the signed well <br /> completion logs are included in Attachment 4. <br /> 2.5 Waste Disposal I <br /> Investigation derived waste (IDW) including soil cuttings and any decontamination water generated <br /> during the well destruction activities were contained in 55-gallon drums and temporarily stored on site. <br /> The IDW was profiled an removed on September 30, 2014 by an approved waste hauler and disposed of <br /> at the Potrero Hills Landfill disposal facility in Suisun, California on October 1, 2014. No waste associated <br /> with this facility remains on-site. A copy of the waste manifest documenting IDW disposal is included as <br /> Attachment 3. <br /> 5 <br />
