This office has not received this work plan. <br /> PHS/EHD was copied on a correspondence dated November 3, 1999 that was sent to <br /> you from the Underground Storage Tank Cleanup Fund Program. The correspondence <br /> requested further documentation be submitted by you in order for the Fund to complete <br /> the initial review of your claim application. <br /> Acceptance of an application to the Cleanup Fund is not required prior to beginning a <br /> PHS/EHD directed site investigation. In fact, compliance with County directives is a <br /> condition of eligibility to the Fund. Even after a site is accepted into the fund, there is a <br /> minimum deductible that must be met by the responsible party. <br /> The work plan is now six weeks overdue. You are requested to contact PHS/EHD <br /> immediately to discuss both your status with the Cleanup Fund and a date for submittal <br /> of the overdue work plan.
