Highway 88 <br /> Stockton, CA 95205 <br /> The San Joaquin County Environmental Health Department (EHD) has reviewed Workplan for <br /> Well Destruction (Work Plan) dated February 27, 2015, prepared and submitted by your <br /> consulting firm Cardno ATC (Cardno) for the subject site. <br /> Cardno proposes to destroy monitoring wells MW-1, MW-2, MW-3, and MW-5 and vapor <br /> extraction well VW-4 by over-drilling the wells using augers with the same diameter as the <br /> augers used to install the wells to remove all installed well materials. Cardno proposes to <br /> destroy MW-4, VW-1, VW-2, VW-3, and VW-5 by pressure-grouting. <br /> The EHD approves the well destruction methods proposed in the Work Plan with the following <br /> conditions: <br /> • Mix neat cement (grout) at a ratio of one 94-pound sack of portland cement with 5 to 6 <br /> gallons of'clean' potable water; <br /> • Remove well boxes prior to destroying the wells; <br /> • Regarding over-drilled wells, fill boreholes with grout, pumped from the bottom of the <br /> borehole up to at least 3 feet bsg through a tremie pipe that extends to within 5 feet of <br /> the borehole's total depth; <br /> • Regarding wells proposed to be destroyed by pressure grouting, enlarge the diameter of <br /> the top 3 feet of the borehole at least one foot larger than the auger originally utilized to <br /> install the well; after the grout mixture has completely filled the casing, apply at least 25 <br /> psi pressure on the grout for ten minutes (destroy the well by over-drilling if the well <br /> casing is unable to withstand this pressure for this length of time); cut off and remove the <br /> casing 6 inches above the bottom of the excavation before inserting grout into the <br /> excavation to create a "cap" with a vertical thickness of one foot; this "cap" must be <br /> installed in one continuous pour immediately after the well has been destroyed; <br /> • All Investigation Derived Wastes (IDW), including all soil cuttings, debris, and <br /> groundwater generated from well destruction activities, are required to be properly <br /> containerized, labeled, stored and disposed; <br /> • A well destruction report describing all well destruction field activities and including the <br /> receiving facility-signed manifests for the disposed IDW must be submitted to the EHD. <br />
