Highway 88 <br /> Stockton, CA 95205 <br /> The San Joaquin County Environmental Health Department (EHD) has received no comments <br /> from interested individuals or agencies regarding the pending case closure at the subject site <br /> during the 60-day public participation period which ended December 25, 2014. Therefore, the <br /> EHD directs that you proceed with destruction of all monitoring and vapor extraction wells <br /> pertaining to the subject site's investigation by submitting a well destruction work plan within 30 <br /> days of the date of this letter. Once the soil vapor extraction remediation system components <br /> and piping have been removed, the wells have been properly destroyed, the well destruction <br /> report has been submitted to the EHD (including manifests signed by facilities that receive the <br /> waste generated during the well destructions), and all fees have been paid to the EHD, this <br /> agency will issue a No Further Action Required letter for the above-referenced site. <br /> After review of the subject site's historical analytical data presented by your consulting firm, <br /> Cardno ATC, the EHD has determined the following wells may be destroyed by the pressure <br /> grout method or over-drill: MW-4, VW-1, VW-2, VW-3 and VW-5. Due to historical presence of <br /> petroleum product and/or fuel additive contaminants; the following wells must be over-drilled: <br /> MW-1, MW-2, MW-3, MW-5, and VVV-4. <br /> To proceed with obtaining the destruction permit for these wells, the following must be <br /> submitted to the EHD to the attention of Gabriel Gonzalez: <br /> • Well destruction work plan, including table(s) detailing well construction (e.g. total depth, <br /> borehole diameter, etc.) and planned destruction method, and a site plan depicting all <br /> well locations; <br /> • EHD Well and Boring Permit Application form; <br /> • Master File Record form; <br /> • Licensed Contractor's Declaration (LCD); and <br /> • A fee of $1,690 (10 x $130 + 1 x $390). The permit fee for the destruction of each well is <br /> $130, along with a $390 Inspection Activity fee. <br />
