In addition to the data normally <br /> presented, the Annual Report shall include the following: <br /> 1. If requested by staff,tabular and graphical summaries of all data collected during the year; <br /> 2. The contents of the regular December monthly monitoring report; <br /> 3. An evaluation of the performance of the wastewater system, as well as a forecast of the flows <br /> anticipated in the next year; <br /> 4. A discussion of compliance and the corrective actions taken, as well as any planned or proposed <br /> actions needed to bring the discharge into full compliance with the waste discharge requirements; <br /> 5. A discussion of any data gaps and potential deficiencies/redundancies in the monitoring system <br /> or reporting program; and <br /> 6. A Water Balance and Capacity Calculation Report must be provided that demonstrates adequate <br /> storage and disposal capacity to ensure full compliance with the WDRs. The water balance shall <br /> evaluate the settling ponds ability to provide sufficient capacity on a month-by-month basis. <br /> Include monthly evaporation,precipitation, percolation, estimated rate of sedimentation, identify <br /> contributions from major sources to monthly discharge volumes, stormwater runoff, and inflow <br /> and infiltration from the surrounding area. Rainfall shall be based on the 100-year return period <br /> total annual precipitation. Note the established maximum daily percolation rate cannot exceed <br /> four to ten percent of the minimum soil permeability and the evaporation rate cannot exceed 80 <br /> percent of the established pan evaporation rate for the area. For the purpose of this analysis, "full <br /> compliance"means maintaining two feet of freeboard in all ponds. Rainfall amounts shall be <br /> based on the total annual precipitation based on a return period of 100 years, distributed monthly <br /> in accordance with historical rainfall patterns. <br /> A letter transmitting the self-monitoring reports shall accompany each report. Such a letter shall include <br /> a discussion of requirement violations found during the reporting period, and actions taken or planned <br /> for correcting noted violations, such as operation or facility modifications. If the Discharger has <br /> previously submitted a report describing corrective actions and/or a time schedule for implementing the <br /> corrective actions, reference to the previous correspondence will be satisfactory. <br /> The transmittal letter shall contain the penalty of perjury statement by the Discharger, or the Discharger's <br /> authorized agent, as described in the Standard Provisions General Reporting Requirements Section B.3. <br /> The Discharger shall implement the above monitoring program as of the ate of this Order. <br /> Ordered by: <br /> Y .
