A video tape of road conditions along the haul route, including but not limited to,Mackville Road from <br /> the entrance is required prior to the start of excavation. The condition of Mackville Road and along <br /> haul route shall be reviewed with the Department of Public Works prior to the start of excavation and <br /> prior to the beginning of each phase. <br /> 4. A bond or cash deposit in the amount acceptable to the Department of Public Works shall be filed with <br /> the Department with a guarantee that, if the permittee fails,refuses or neglects to repair, under permit, <br /> any damages to the haul route, including but not limited to,Mackville Road which may result from <br /> the permittee's operations,the Department of Public Works may do the work necessary to restore <br /> Mackville Road equal to,or better than, its original condition, using the funds from the bond or cash <br /> deposit. <br /> 5. The Traffic Impact Mitigation Fee will be required for this development. The Fee is due and payable <br /> at the time of excavation plan approval. The Fee currently is $151.36 per additional daily trip-end in <br /> the Lockeford area. The Fee shall be automatically adjusted July 1 of each year by the Engineering <br /> Construction Cost Index as published by the Engineering News Record.
