Q/� <br /> PHONE NUMBER: (209) 468-3455 77V�/ <br /> RE: PA-04-251 (SU,Pre Application),Attention: Jennifer Jolley <br /> 9422 East Jahant Road,Acampo <br /> The Environmental Health Department has the following concerns on this pre-application that <br /> need to be addressed: <br /> qualified environmental professional shall prepare a surface and subsurface contamination <br /> report, identifying any potential source of surface or subsurface contamination caused by past <br /> or current land uses. The report shall include evaluation of non-point source of hazardous <br /> materials, including agricultural chemical residues, as well as potential point sources, such as <br /> fuel storage tanks, septic systems, or chemical storage areas. (San Joaquin County <br /> Development Title, Section 9-905.12(a)). In addition,the report shall be submitted to the <br /> Environmental Health Department at the time of submittal of a tentative map. <br /> B. Environmental Health Department requires a soil suitability/nitrate loading study <br /> incorporating proposed staff and customer use, indicating that the area is suitable for septic <br /> tank usage. This must be performed prior to approval of a Parcel/Final Map (San Joaquin <br /> County Development Title, Section 9-1105.2(d)). A nitrate loading study is required, and a <br /> review fee of$465.00 must be paid at time of submittal to Environmental Health <br /> Department. <br /> The sewage disposal system shall comply with the on site sewage standards of San Joaquin <br /> County prior to approval. A percolation test that meets absorption rates of the manual of <br /> septic tank practice or E.P.A. Design Manual for onsite wastewater treatment and disposal <br /> system is required for each parcel. A permit fee of$93.00 per percolation hole is required. <br /> Page 1 of 2 i- �1_ (�1 <br />
