Surface and Subsurface Contamination Report- APN: 009-100 - 02 - PA-04-102 (MS) Service Request No. 35368 -To create four(4) parcels containing 80 acres within an existing 320 acres April 26, 2004 San Joaquin County Environmental Health Department The inclusion of discussion or testimony of previous owners in this addendum is provided in response to letter from Donna Heran, Director, dated April 19, 2004. In the past I have included an environmental questionnaire filled in by the property owner in my Surface and Subsurface Contamination Report but in this particular case one was not included due to some extenuating circumstances. In this case I was contracted to prepare a Surface and Subsurface Contamination Report by the Leeward Investments, LLC company, addressed at One California Street, Suite 300, San Francisco, CA 94111, phone: (415) 284- 0779, Fax (415) 284-0784, through their representative, Kent M. Rowett. At the time I prepared my contamination report the company was in the process of trying to complete the purchase of the grape vineyard. I submitted the environmental Questionnaire to Mr. Kent Rowett to fill in and return to me. When I contacted Mr. Rowett to find out why the questionnaire was not returned with the signed contract for preparing the contamination report, he informed me that the property owner for some unknown reason was being unfriendly and uncooperative and was not willing to provide him with any present or past history on the property they were in the process of acquiring and since he had no personal knowledge in this regard he was not able to provide the information that had requested in the questionnaire. Therefore, I was unable in the past and am not able presently to provide either the environmental questionnaire or other discussion or testimony from the then property owner. Sincerely, Walter E.
