HIRATA <br /> DIRECTOR <br /> August 6, 1998 <br /> Mr. Gary Herd <br /> Post Office Box 488 <br /> Woodbridge, California 95258 <br /> SUBJECT: WATER SERVICE FOR ASSESSOR'S PARCEL NO. 015-150-37 <br /> (PARCELS ONE AND TWO OF LA-98-34)FROM MOKELUMNE ACRES <br /> MAINTENANCE DISTRICT a� �' LAac_ <br /> Dear Mr. Herd: <br /> The Mokelumne Acres Maintenance District will service the above referenced site with water, <br /> provided all necessary improvements are made to the water system which comply with State and <br /> County Standards. <br /> Annexation to the Mokelumne Acres Maintenance District will be required and all applicable <br /> connection or buy-in fees shall be paid. A Statement of Annexation Costs is attached. Please <br /> provide a copy of the Grant Deed of the property to be annexed. <br /> If any extension, connection or placement of new facilities is required to connect to the existing <br /> supply,the cost shall be borne by the applicant. An improvement plan shall be submitted for <br /> review and approval. A water meter(Sensus brand measurable in gallons) shall be installed and <br /> provided with attachment connector spud. Any interruption to the existing water system shall be <br /> done by the Utility Maintenance Division only. If any interruption to the existing water system <br /> is necessary,please contact the Utility Maintenance Division at 468-3090 to perform the task. In <br /> addition, the Utility Maintenance Division shall be contacted prior to backfilling. <br /> If the existing well is to be destroyed, a permit from the Environmental Health Division shall <br /> be acquired before water will be supplied. Destruction of the well shall be completed within <br /> 30 days of hook-up. If a well is to remain on the property, an approved reduced pressure <br /> backflow prevention device will be required. The applicant shall have the reduced pressure <br /> backflow prevention device tested by a licensed tester prior to final inspection and every <br /> year thereafter at the owner's expense. A copy of the test result shall be submitted to the <br /> Utility Maintenance Division. <br />
