individual thicknesses of sand beds or layers that are anticipated to take injection fluids (5) physical and chemical characteristics of the forma- tion fluids C) Confining layer information (1) formation fluid pressure (2) other physical and chemical characteristics of the confining zone matrix including: - hydraulic conductivity, vertical and horizontal porosity - drill core and sidewall core analysis - thicknesses of significant shale layers or beds (3) physical and chemical characteristics of the forma- tion fluids D) Maintenance and calibration of monitoring equipment Prior to commencing injection the permittee shall submit to the Director, for review, procedures to be used for the proper operation and maintenance of the injection well . These procedures shall include a schedule for calibrating equipment, updating charts, regular main- tenance of the well and system, and other related main- tenance and monitoring checks. The Director must be notified of any changes in the above procedures. E) Continuous monitoring devices. The permittee shall install continuous monitoring devices to monitor temperature; injection pressure, rate, and volume; and annular pressure in accordance with Part II, Section C, of this permit. The equipment shall be installed and operating prior to commencing injection of waste fluid into the injection well . F) Automatic alarms and shut-off devices. The permittee shall install automatic alarms and shut-off devices which will immediately notify the operators of a malfunction in the event that the injection pressure at the wellhead exceeds the maximum injection pressure authorized by this permit. The automatic alarms shall also notify the operators of a significant change in the annular pressure in accordance with Part II, Section B, item 6, of this permit. 23
