You are requested to complete the enclosed Report of Waste <br /> Discharge for the pretreatment facilities you plan to construct to treat your <br /> wastewater prior to injection in the well . Please include plans for sludge <br /> disposal , spill prevention, and a back-up system in case the injection well <br /> fails or is down for maintenance. <br /> When we receive the completed Report and additional information, we will <br /> evaluate whether or not waste discharge requirements for your pretreatment <br /> facility will be necessary. <br /> Please submit the Report of Waste Discharge and additional information to us <br /> by 13 April 1990.
